We’re a little old-fashioned in our approach because we want to get to know you and your family. We value the trust you place in us and the relationship we will build with you over the years.
If you’d like to become a Saltwater Medical patient, please call us on 07 5301 9828 us to make your initial appointment. All new patients will be booked into a long appointment for their first consultation when joining our practice, as this allows your doctor to take a thorough history and work through your current health concern. We ask all new patients to arrive at least 10 minutes prior to their appointment to allow our staff time to set up your file and provide you with a brief orientation of our practice. If you prefer, you can download and fill in our new patient form below, prior to your initial appointment. When you arrive please give your completed form to our receptionist.

Please note the following important practice policies:

Drugs of addiction – Our doctors will not prescribe S8 drugs or drugs of addition at a new patient’s first appointment with our practice, nor will they ever provide prescriptions for these medications going forward without an appointment. Non-attendance / late cancellations / punctuality – We take pride in delivering high quality healthcare, which is why your appointment time is booked and held especially for you. If you no longer require it, or cannot attend, we require at least 4 business hours’ notice of your cancellation. We are usually fully booked, so giving 4 hours’ notice allows us to offer your appointment to other patients. Failure to attend or notify us within the required time frame will incur a non-Medicare rebatable fee ($82 for a standard appointment, $146 for a long appointment). Repeated late or non-attendance may also result in the inability to book further appointments. We pride ourselves on running on time (barring medical emergencies), so your punctuality is also appreciated. If you arrive unreasonably late for your appointment you may need to reschedule.
